Whether you are in your 20s or your 80s, Advance Care Planning (ACP) is about making decisions ensuring that your wishes and preferences are honoured should the unexpected happen or you’re unable to communicate.

What is Advance Care Planning?

Advance Care Planning (ACP) allows individuals to reflect on their values, wishes and care preferences, ultimately making a plan for their care. And care comes in many forms. ACP is more than just making decisions about your medical care. It is about planning now so you can rest easier, and spend more time making memories with your loved ones.

When the unexpected happens, there is no greater gift you can give than letting your loved ones and caregivers know what is important to you.

Learning from others, especially experts, makes it easier to understand what might be important to include in your plans so that your individual wishes are clear. You may choose to express your wishes simply, or to provide as much detail that suits you and your unique life.

Your life story is important. And no matter what chapter you are in of that story, it's important to share it with loved-ones. Planning is about expressing what is important to you, your expectations and your hopes.
